Mandatory Forms

Before your child is enrolled in a program at The New Generation, the Department of Human Services requires a few forms to be completed. There will be no exceptions; the paperwork must be completed for our programs.

These forms included the emergency contact that must be completed and updated every 6 months. There is also a child health assessment that must be completed within the first 30 days of enrollment and updated yearly. Enrollment is available on a full-time or part-time basis.
